To evaluate the clinical efficacy and safety of photodynamic therapy (PDT) plus tea polyphenols (TP) in treating peri-implantitis.
The clinical records of 91 peri-implantitis patients treated at the North China University of Science and Technology Affiliated Hospital and Tangshan Hongci Hospital were analyzed retrospectively. Forty-one patients who received PDT alone were assigned to the control group, while the other 50 patients receiving both PDT and TP were allocated to the study group. Periodontal indices, inflammatory factors, swallowing function, pain, MOS 36-Item Short-Form Health Survey (SF-36) scores, therapeutic effect and adverse reactions were compared between the two groups.
Post therapy, significant reductions in the modified plaque index (mPLI), modified sulcus bleeding index (mSBI), probing depth (PD), and clinical attachment level (CAL) were observed in both groups (P<0.05), with more notable decreases in the study group (P<0.05). Besides, the levels of tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) as well as interleukin-1β (IL-1β) also decreased notably in both groups (P<0.05), with notably lower levels in the study group (P<0.05). SF-36 scores across all dimensions significantly improved in both groups (P<0.05), with the study group demonstrating more substantial improvements (P<0.05). Swallowing function scores significantly decreased in both groups (P<0.05), with a more marked improvement in the study group (P<0.05). Similarly, the VAS scores of both groups significantly decreased post-treatment (P<0.05), especially the study group (P<0.05). In contrast to the control group, a notably higher overall response rate was observed in the study group (P=0.020), while no significant inter-group difference was observed in the total incidence of adverse reactions (P=0.683).
PDT plus TP is more effective than PDT alone in treating peri-implantitis. This combined approach significantly reduces inflammation, improves periodontal health, and enhances quality of life, without increasing the incidence of adverse reactions. These findings suggest its potential for broader clinical application.
评估光动力疗法(PDT)联合茶多酚(TP)治疗种植体周围炎的临床疗效和安全性。
回顾性分析华北理工大学附属医院和唐山弘慈医院收治的91例种植体周围炎患者的临床记录。41例仅接受PDT治疗的患者被分配到对照组,另外50例同时接受PDT和TP治疗的患者被分配到研究组。比较两组的牙周指数、炎症因子、吞咽功能、疼痛、医学结局研究简表36项健康调查(SF - 36)评分、治疗效果和不良反应。
治疗后,两组的改良菌斑指数(mPLI)、改良龈沟出血指数(mSBI)、探诊深度(PD)和临床附着水平(CAL)均显著降低(P<0.05),研究组下降更明显(P<0.05)。此外,两组的肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)以及白细胞介素-1β(IL-1β)水平也显著降低(P<0.05),研究组水平更低(P<0.05)。两组各维度的SF - 36评分均显著改善(P<0.05),研究组改善更显著(P<0.05)。两组的吞咽功能评分均显著降低(P<0.05),研究组改善更明显(P<0.05)。同样,两组治疗后的视觉模拟评分(VAS)均显著降低(P<0.05),尤其是研究组(P<0.05)。与对照组相比,研究组的总有效率显著更高(P = 0.020),而不良反应总发生率组间差异无统计学意义(P = 0.683)。
PDT联合TP治疗种植体周围炎比单纯PDT更有效。这种联合方法能显著减轻炎症,改善牙周健康,提高生活质量,且不增加不良反应发生率。这些发现表明其具有更广泛临床应用的潜力。
